Dr. Gregory Gullo, founder and Medical Director is a fellowship-trained Interventional Pain Medicine specialist with board certifications in both Anesthesiology and Pain Management. He has been practicing medicine for approximately 35 years, focusing on Pain Management for more than 19 years. Prior to relocating to the Portland area, Dr. Gullo was Assistant Professor of Anesthesiology at Penn State University’s Milton S. Hershey Medical Center, where he supervised the Pain Management and Palliative Care Center. Later he entered into private practice in the Palm Springs, CA, area before settling in Northwest Oregon in 2005. He founded Willamette Pain and Spine Center in 2006.
Dr. Anish Mirchandani is a fellowship-trained Interventional Pain Medicine physician with residency training in Physical Medicine and Rehabilitation. Dr. Mirchandani completed his residency training at Columbia and Cornell University in New York, followed by an ACGME accredited Pain Medicine fellowship at the University of Texas, San Antonio. Dr. Mirchandani is experienced in providing a comprehensive and multidisciplinary treatment plan that is uniquely individualized to restore function and improve pain. Using specific interventional techniques, physical therapy, musculoskeletal ultrasound, Dr. Mirchandani treats conditions affecting the spine, peripheral joints, and other chronic painful conditions. He also has a special interest in the clinical application of neuromodulation for chronic pain conditions. Dr. Mirchandani joined Willamette Pain and Spine Center in September 2018. He is excited to embrace the beautiful Pacific Northwest.
Shae Montegna, PA-C is a native Oregonian. In 2000 she graduated from the Oregon Health and Sciences University, Physician Assistant Program, located in Portland, Oregon. She has clinical experience in Family Medicine, Urgent Care, and specializes in Pain Medicine. In 2005, Shae assisted with disaster and humanitarian relief in rural Zimbabwe while practicing medical and surgical care. Shae stays busy with her husband, Sam, and her 3 sons. She values physical fitness as she is a runner and enjoys weight training. She is recharged by hiking, traveling abroad, and listening to local live music.
Dale Lee, FNP, is a nurse practitioner for Willamette Pain and Spine Center. He graduated from the Family Nurse Practitioner program at the University of Alabama at Birmingham School of Nursing in 2017 and holds the Master of Science in Nursing. He also earned his Associate of Science in Nursing at Central Oregon Community College in 2012, and his Bachelor of Science in Nursing at Auburn University at Montgomery in 2015. He also carries post-graduate degrees in Chinese medicine and naturopathy from the National University of Natural Medicine in Portland, as well as the Bachelor of Science in Biochemistry from the University of Washington, in Seattle. Prior to becoming a healthcare provider at Willamette Pain and Spine Center, he worked as a staff nurse and surgery nurse at Baptist Medical Center in Montgomery, Alabama, from 2012 to 2017. An Oregon native, Dale was born and raised in Bend, Oregon, and has lived in the Pacific Northwest for much of his life. He enjoys practicing Iaido, the martial art of the Japanese sword, and is learning to play the alto saxophone.

The Medical Assistants here at Willamette Pain and Spine Center act as liaisons between you and your Healthcare Provider and help to meet any needs you may have between appointments!
New Patient Coordinator
Our New Patient Coordinator is the person who helps you schedule your first appointment. She is responsible for processing all new patients into our system, verifying insurances and obtaining referrals and records from your primary care physician if needed. She can be reached by choosing option 5 from the main phone prompt.
Injection & Procedure Coordinator
If you have any problems getting an insurance company to approve an injection or procedure, reach out to our Prior Authorization Coordinator at option 0.
