Effective March 1st, 2019, we can no longer treat Medicaid patients at Willamette Pain and Spine Center. Although we have not been direct Medicaid providers in over 4 years, we have been able to help in the treatment of commercial patients under the Oregon Health Plan. The guidelines have changed and the Oregon Health Plan will no longer cover office visits or care as we are not in-network.

What this means is that we will need to refer patients back to their Primary Care Providers (PCP) as the Oregon Health Plan requires that a new referral be submitted via the PCP to become established elsewhere. Please feel free to read the letter here that we’ve provided to our patients explaining why and answering some common questions. Thank you for your understanding during this time of transition.

  • MODA Health (formerly ODS) ( We are OON with OHSU PPO but still accepted)
  • Pacificsource (if Legacy Plan/Network, Only contracted with Legacy+ Employee Plan.)

  • Providence, PPO, Medicare**, and Providence Connect (with an insurance referral)
  • Prime Health

  • Triwest (with an insurance referral)

*Although Willamette Pain and Spine verifies eligibility and benefits, it ultimately is our patients’ responsibility to verify network participation with your group health plan. Although the list above is current, there are many sub-networks that Willamette Pain and Spine is not participating in.

If you are an existing patient who does not have current insurance coverage, the self-pay price is as follows:
Follow-up visit: $220

We do not see new patients without a physician referral.
